New Membership Form

A new membership form with better layout has been created for the 2012 MIA membership. Please use this new form only.

Note: MIA memberships expire at end of December with a grace period of end of February as per article III, Section 2, (b) of the MIA constitution. Also, old MIA memberships forms will still be accepted.
